NDIS Psychology Referral Form
Use this form to refer a plan-managed or self-managed NDIS participant for psychology support. Please provide only information relevant to assessing the referral and arranging suitable services.
What NDIS-funded psychology may support
NDIS-funded psychology must relate to the participant’s disability, functional capacity and NDIS goals. Depending on the participant’s needs and approved supports, psychology may assist with emotional regulation, coping skills, social participation, routines, behaviour, communication, independence and capacity building.
The NDIS does not generally fund clinical treatment whose primary purpose is diagnosing or treating a health condition. Where the participant’s main need is clinical mental-health treatment, another pathway such as Medicare, private healthcare, a GP, psychiatrist, hospital or public mental-health service may be more appropriate.
Our intake team will review the referral information and discuss the most appropriate service pathway. Submission of a referral does not guarantee that the requested support is eligible for NDIS funding or clinically suitable.

Funding and fees
Therapy Near Me currently accepts plan-managed and self-managed NDIS participants for eligible services. We do not currently accept NDIA-managed funding.
Applicable session fees, report writing, non-face-to-face work, travel, cancellations and other billable supports will be explained before services commence and documented in the participant’s service agreement.
Charges will not exceed applicable NDIS price limits where those limits apply.
Working with the participant’s support team
With the participant’s or authorised representative’s consent, Therapy Near Me may communicate with support coordinators, plan managers, families, guardians, GPs and other treating professionals where this is relevant to assessment, service delivery, reporting or continuity of care.

Who can submit a referral?
The participant, an authorised representative, family member, guardian, support coordinator, recovery coach, plan manager or health professional may submit a referral. The person submitting the form must have appropriate authority or permission to share the participant’s information.
Do you accept NDIA-managed participants?
No. Therapy Near Me currently accepts plan-managed and self-managed NDIS participants. We do not currently accept NDIA-managed funding.
Is the participant guaranteed an appointment?
No. The intake team first reviews service suitability, risk, goals, funding, location, practitioner scope and availability. We will explain the available pathway after reviewing the referral.
Can psychology be delivered by telehealth?
Yes. Telehealth psychology is available Australia-wide where it is clinically suitable and appropriate for the participant. Local in-person services may also be available in selected areas.
Can you provide home, school or community appointments?
These appointments may be available in selected locations. Availability depends on practitioner location, clinical suitability, travel requirements, safety, participant preference and funding.
What information should be included?
Provide the participant’s contact details, funding type, plan dates, disability-related functional impacts, NDIS goals, requested support, delivery preference, communication needs and relevant risk or safety information. Upload only documents relevant to assessing or delivering the requested service.
Can the NDIS fund general mental-health treatment?
NDIS funding is generally directed to disability-related supports and capacity building rather than clinical treatment whose primary purpose is treating a health condition. Our intake team can discuss whether the referral appears suitable or whether another healthcare pathway may be more appropriate.
What happens after submission?
The intake team reviews the referral and contacts the nominated person within one business day to discuss suitability, funding, practitioner availability and the next steps. Complex referrals may require further information before a service can be offered.
What should I do in an emergency?
This referral form is not an emergency or crisis service. If there is immediate danger or a life-threatening emergency, call 000. For urgent mental-health support, contact the participant’s treating team, local crisis service or an appropriate national crisis service.
